From the description of the Gate spell in the Greyhawk Supplement (1975):
"Employment of this spell opens a cosmic portal and allows an ultra-powerful being (such as Odin, Crom, Set, Cthulhu, the Shining One, a demi-god, or whatever) to come to this plane"
Earliest reference to the Cthulhu mythos in D&D?
Probably but the context was obviously meant to be a generic reference to any extraplanar being from a variety of mythos. Not sure if it counts..
