I've been looking for a hard copy of this magazine ever since I read that pdf some years ago. I finally got one today and I was certainly surprised to discover this illustration, not found in the pdf!
We were both pleased with that illustration. If you look carefully the monsters are playing with figures of doctors. A reference to his group of medical students. We, high school students also played with a couple of his favorite med students and we all got along fine.
Took me a bit to track it down, but PDF of the article available here: https://docs.google.com/viewer?url=http://dl.dropbox.com/u/14957979/ConfessionsOfADungeonMaster1980.pdf
